OK, I guess my question is to what address do I send my complaint? I could not find a link on the DOL WHD page.

The FMLA is a federal law. If your FMLA rights have been violated, you may file a complaint with the federal Department of Labor.

Yes, your employer absolutely may require you to explain why you're absent and/or why you are taking FMLA. Among other reasons, they have an obligation to determine if the absences qualify for FMLA. There is also a FMLA certification form they may require you and your doctor to complete which includes a diagnosis/reason for taking FMLA.

I know I can file a FMLA complaint with the Wage and Hour Division, but I'm not sure exactly where to file. Also, is there a form or guidelines as to what to include in a complaint. Finally, does anyone know if you have to specifally say that you are taking FMLA leave when you call-in or schedule a surgery or whatever (assuming it's for one of the things covered by FMLA?
