What is an 'Information subpoena?

If I am starting a lawsuit against someone for the first time; can I request it when filling out 'statement of claim/complaint form'?

Can I request for my filling and service fees be tacked on to suit?

If so, then do I request it during initial suit?

Do I need to pay out of pocket if I chose to request a jury trial when filling out 'statement of claim'?

Must you be represented by an attorney if you request jury trial?
